Duplicate Driving License

In the event of loss or theft an application for duplicate license can be made in the prescribed manner along with a report from the police. In the case of old license being destroyed, damaged or defaced, an application for duplicate can be made on the surrender of the license.

An application for Duplicate Driving License has to be submitted in Form 9a accompanied by the following :

  • Detail of Driving License.

  • Recent Passport sized photograph – 3 copies.

  • Police Report.
